    Are mathematics majors required to take a history of mathematics course? Also, if not, are there are universities that offer such courses?

    I know of no university that requires such a course.

    Here is a web site that lists several university courses in the history of mathematics. It is not up to date. I don't know how far out of date it is, but I do know that Jim Carlson, whos is listed as one professor offering such a course, has not been at the University of Utah for several years. He is now the president of the Clay Mathematics Institute in Boston.
