    Hi, I'm writing a story and would like to have what appears to be an authentic psychiatric record of a patient. I don't know what it should look like though. Any help would be appreciated, I'm using search engines but am having a hard time finding a template.

    That will do, thanks. Is there a version without the "sample" stamp?
